I blame karine!!! you did not use ass or fuck once in this story even when it could have been quite appropriate. Anyways i found the story rather disturbing, first time i witness tuxedo fetish. The humour was most beneficial to your story as a whole and quite educational (i learnt that mastoidectomy meant brain surgery).
All in all not bad just a little too superficial at certain parts where it shows that you are trying to hard for a refined vocabulary and you are not being yourself.
